By signing this mandate form, you authorise PREMIER DATA NETWORKS to send instructions to your bank to debit your account and your bank to debit your account in accordance with the instruction from PREMIER DATA NETWORKS LTD. As part of your rights, you are entitled to a refund from your bank under the terms and conditions of your agreement with the bank.
I understand I must take the service for a minimum term of 24 months. After this term I can cancel the service by giving 30 days notice. A cancellation fee of the remaining months in the contract will be applied if I cancel within the minimum term. I declare I am over 18 years of age and that I am the owner of the premises or I have permission to install the equipment onto the premises from the owner. I understand that by using this account I have agreed to the terms and conditions/acceptable usage policy and fee schedule available on our website. The installation charge if applicable is non-refundable. The equipment remains the property of Premier Data Networks at all times. A charge of 100 EUR will be applied to the account if the equipment is not returned at the termination of this contract. A missed appointment charge of 50 EUR is applied if you fail to be at the address at the agreed time, or you cancel the installation 48 hours before or on the day of installation. If your order after the PREORDER date an installation charge of 100 EUR will apply. Please contact our staff for preorder date information.
